A Clinical Study on 111 Cases of Colorectal Cancer


Abstract
In Korea, the rate of incidence and mortality of colorectal cancer has been increased, and many clinical analyses have been done at many hospital.
III cases of colorectal cancer were analysed and described in view of the general concepts during the last 6 years from January 1984 to December 1989 treated surgically at Department of General Surgery, Daejeon Eulji General Hospital.
@ES The results were followings;
@EN 1) The ratio between male and female was 1.52 : 1 and age incidence was the most prevalent in the 7th decade(36.1% of total cases).
2) The more frequent symptoms and signs were abdominal pain(88.9%), weight loss(29.6%) and palpable mass(18.5%) in the right colon cancer, abdominal pain(90.5%), melena(42.9%), bowel habit change(38.1%) and palpable mass(33.3%) in the left colon
cancer, Melena or bloody stool(82.5%), bowel habit change(57.1%), tenesmus(49.2%) and abdominal pain(39.7%) in the rectal cancer.
3) 94.6% of rectal cancer was diagnosed initially by digital examination. 98.0% of rectal cancer and 94.7% of colon cancer were diagnosed by colon study, 96.6% of rectal cancer and 81.8% of colon cancer were diagnosed by colonofiberscope and
sigmoidofiberscope. 100% of rectal cancer and 85.7% of colon cancer were diagnosed by C-T scanning.
4) The duration of the symptoms and signs prior to admission was the most cammon below 3 months(42.4% of total cases).
5) Location of the tumor was the most frequent in the rectum(56.8%), sigmoid colon(10.8%) and ascending colon(10.8%).
6) Miles' operation(36.9%) was performed the most frequently, right hemicolectomy(21.6%) being the next and total resectability was 85.6%.
7) The most frequent macroscopic finding of colorectal cancer was annular type(57.7%).
8) the most frequent histopathologicl type was adenocarcinoma(98.2%).
9) The mesenteric lymph node metastasis of colorectal cancer was 54.1%. The most common distant metastatic site of colorectal cancer was pelvic organs(10.8%), and liver(8.1%) being the next.
10) The stages of colorectal cancer were modified Dukes' classification, A in 2.7%, B in 36.9% C in 45.0% and D in 15.3%.
11) Postoperative complications were developed in 32.4% of the total cases and the most frequent one was wound infection(12.6%).
12) Operative mortality was 0.9%.
